in one of our forms. A parent must sign it in front of a notary public.
The parent must come in with the minor and the notarized form. There are
separate forms for tattoos and piercings which you can print out from this
site. We encourage parents to ask questions and make informed decisions
regarding their children. To get the forms, click the links below. They

Can I get AIDS from a tattoo?
- According to the Centers for Disease Control (CDC), there has NEVER been
a tattoo-related case of AIDS.
-
What about hepatitis?
- The CDC has not attributed a case of hepatitis to a tattoo shop in over
a decade. While it is certainly possible to transmit hepatitis through tattooing
and piercing, it is very easy to prevent this from happening. Go to a reputable
shop that practices proper sterile procedures.
-
What about hepatitis C?
- There was a media buzz several years ago linking tattooing with a high
rate of hepatitis C transmission. This was based one small and very flawed
study, relying on very old numbers. The CDC has studied the issue using
very large and statistically valid populations, and their official position
is that "No data exist in the US indicating that persons with exposures
to tattooing and body piercing alone are at increased risk of HCV infection."
-
What do you do to prevent disease transmission?
- All needles, both tattooing and piercing are single-use. Other items such
as clamps used in piercing and tubes used in tattooing are autoclaved. The
autoclave is monitored monthly to insure it is working properly, and results
are posted on the wall. Gloves are worn for every procedure. We practice
Universal Precautions and are in compliance with all Department of Health
regulations. We are happy to answer any questions you may have about this.
We strongly believe that an informed public is our best customer.
